Community Nursing Care

This is the provision of specialist care for participants who have high care needs requiring a high level of skill, and for the training of support workers to respond to the participant’s complex needs such as:

.Wound care and dressing changes

.Medication management (including injections)

.Catheter or stoma care

.Ventilation and tracheostomy support

.PEG feeding support

.Diabetes management (including insulin injections)

.General health monitoring and advice

.Developing a care plan for support staff

Community nurses can visit your home, or you may receive care at a local clinic depending on your situation.

✅ What is This Support?

Community Nursing Care provides specialised care from a registered nurse to help you manage health needs related to your disability. It supports you to stay healthy and safe at home or in the community.

This support is for people who need clinical or complex health care that a regular support worker can’t provide.

You might need Community Nursing Care if:

You have a disability-related health condition that requires clinical care
You need support with complex medical needs at home
A support worker is not trained to safely provide the care you need

📝 How to Access This Support

Speak to your NDIS planner or support coordinator.
Make sure “Community Nursing Care” (0114) is included in your NDIS plan.
Choose a registered provider who offers nursing care.
The nurse will assess your needs and create a care plan if required.
